From another list-serve that I'm on: "Use these forms to quickly document observations of behaviors. Such observations can be a good first step in a functional behavior assessment or analysis. I have DSL, and the first one took about a minute, so I know it would be slower if you have dial-up.

BTW just for your info I am still in the dark ages LOL and have dial up and the documents came up 1-2-3 for me ;D

OMG Laura thank you thank you this post couldn't have came at a better time. We have my oldest nephew with us right now who was just discharged from the hospital following a major mental breakdown at school. The docs and therapists have no idea what is going on with him right now and with these charts I can document his behavior here and hand them over to his docs. Of course for some reason he does better over with us so we won't see the major behavior problems everyone else does but at least if we do I can immediately fill in the blanks of these papers.

Thanks Laura! I actually found it to be interesting. We were just talking about behaviors in general at the Back To School night at Russell's school on Tues. There were about 6 parents there and our teacher. I sent it in an email to her.

Laura -- how great and timely!! Meghan has been a bit, I don't know how to describe it...disobedient, extra-pouty, etc lately. I don't know if it's because she recently started in full day preschool and isn't napping as much, or because she's becoming more jealous of Matthew. Or something else - like not seeing her grandma (her best friend) as much... anyways...this is terrific because it's just what I needed right now! thanks - laurie

;D Those are great!!! We had one in kinder we used. We would usually look at one behavior like Olivia would not step foot on the kinder apparatus. So as a team we would all have to observe Liv seperately then write down the thing that preceded the behavior(ie. like her laying on the floor) and what the end result was. It took a second and wasn't too hard since the 3 kindy teachers her aide me her sp ed teacher the inclusion facilitator the principal the st and ot all came to these weekly 20 minute meeting. It's UNBELIEVEABLE what groups can resolve. We brasinstormed the following week came up with a plan based on each of our individual findings and VOILA the neg beh was extinguished so quickly. Its amazing what collaboration in the school setting can do for a special needs child.
