I have called for an IEP addendum meeting and below are notes I've made to bring to the meeting next week. His Regional Center case worker will be attending with me, thank goodness. I'm listing these notes to see if you all may have a better way I should word things or ideas relating to each topic that I might have missed. I'm still nervous when I go to these meetings and although it might not show when I'm there, I'm still a nervous wreck! I usually can't sleep the night before!
Any ideas or help rewording something is appreciated. Thankyou!!!

1. not a life threatening issue...but Jelani is listed on first page of IEP in the ethnicity codes section as African American. This is suggesting he is black only which isn't true, I am his biological mother and I am Caucasion. This makes Jelani African/Caucasion. Can we please make the correction on IEP?
2. On first page where it states Parent/Guardian, I would my name only to be listed and not his father's name. I am Jelani's sole guardian and unfortunate to say, Jelani's father is absent and is in no way involved in his son's life. If possible please correct this and/or have a separate line indicating that Jelani lives at home with his mother. Please makes changes on IEP.
3. Jelani's listed as MR under diagnosis...which I understand to be needed for qualifying him for services, but nowhere is it indicated that Jelani has Down syndrome. I would like that Down syndrome be indicated somewhere in IEP.
4. Jelani is visually impaired. His glasses need to remain on his face at all times as he is virtually blind without them. Please indicate this in IEP.
5. Jelani has hearing loss and the sedated Auditory Response test was cancelled because he's been ill. The hearing test will be rescheduled in April. Many times it's been thought that Jelani doesn't respond to his name or when asked to do a given task, while this may be true in some instances we can't rule out the possibility that he can't hear well enough, and not assume that it's only behavioral when he doesn't respond. Jelani has sensorineural hearing loss and the amount and suggestion of hearing aids will be understood after hearing test in April.
6. On Page #3 of IEP "Communication Development"
Here it states quote/unquote "Jelani is yet imitating sound or words when requested or by imitation". This is not a true statement. It should read that Jelani "is" imitating sound/words and while not consistently, he is never the less imitating. Please correct this in IEP.
7. Communication book needs to be written in IEP please.
8. Diaper changes/potty time 2x daily. On several occasions he has come home soaked. Please include above in IEP.
9. Jelani is to be hand held at all times when climbing stairs, ie: when boarding and exiting bus because of the fall risk due to his low muscle tone. Please indicate this in IEP.
10. I would like a written report of Jelani's progress given to me once a month from his Speech Therapist and Physical Therapist. I need to open the lines of communication between parent and Therapist so that I can better assist Jelani at home. I want to be able to follow-up with what is being worked on in school, and maybe share some ideas with me. As we all know, Jelani has severe speech delay and very low muscle tone. I've been told that he often trips and sits down alot when he shouldn't. I would like to know ways I can assist Jelani in strengthening his muscles and being more verbal. There is no such thing as "knowing too much". I need updates.
Please have these written reports made once a month indicated in IEP and put in his backpack.
